AnnaC I'm sorry to hear you are still experiencing issues. I've checked in with our form provider and it seems that there are not any issues we can fix on our side. Can you share what operating system (i.e., Mac, Windows, iOS, etc.) and browser (Safari, Google Chrome, Microsoft Edge, etc.) you are using to fill out the form? If you have access to another device or browser (including a smartphone or tablet), it will probably be the quickest to try submitting the form from there. We are very sorry for the inconvenience.
